Whole-Person Health Care,
Rooted in Community
Catamount Health is a 501(c)(3) nonprofit community wellness center providing accessible, evidence-based primary and preventive care to individuals and families throughout Washington County and central Vermont. We believe every person deserves high-quality health services regardless of income, insurance status, or background. Our integrated care model connects preventive screenings, behavioral health counseling, chronic disease management, nutrition therapy, and community education under one roof, making it easier for patients to achieve lasting wellness.
“Our mission is to advance health equity across central Vermont by delivering compassionate, culturally responsive, and evidence-based care that addresses the whole person—body, mind, and community.”
— Dr. Margaret Holloway, Founder & Executive Director
Comprehensive Health Programs
Our patient-centered medical home brings together multiple disciplines so you receive coordinated, continuous care tailored to your unique health goals. Every program is delivered by licensed, board-certified professionals committed to the highest standards of clinical excellence and compassionate service.
Preventive Wellness Screenings
Annual physicals, biometric panels, cardiovascular risk assessments, age-appropriate cancer screenings, and immunization tracking. Our USPSTF-aligned protocols ensure you receive every Grade A and B recommended screening on schedule, catching risk factors early when intervention is most effective.
Learn More →Behavioral Health & Counseling
Licensed clinical social workers and counselors provide individual therapy, group support sessions, substance use disorder treatment, and crisis intervention. We integrate PHQ-9 and GAD-7 validated screening into every primary care visit, closing the gap between physical and mental health.
Learn More →Chronic Disease Management
Evidence-based care coordination for diabetes, hypertension, COPD, heart failure, and other ongoing conditions. Our team uses remote patient monitoring, medication therapy management, and structured self-management education to help you take control of your health between visits.
Learn More →Nutrition & Dietitian Services
Registered dietitians offer medical nutrition therapy (MNT), weight management counseling, diabetes-specific meal planning, and community cooking education. We partner with local farms and food banks to address food insecurity as a core social determinant of health in rural Vermont.
Learn More →Maternal & Prenatal Wellness
Comprehensive prenatal and postpartum support including wellness visits, gestational diabetes screening, lactation counseling, and perinatal mood disorder screening. Our certified nurse-midwife collaborates with regional OB-GYN specialists to ensure continuity of care from conception through the first year.
Learn More →Community Fitness & Movement
Free and low-cost group fitness programs including the Move More Vermont walking group, chair yoga for seniors, supervised cardiac rehabilitation exercise, and family wellness classes. These programs are open to community members regardless of patient status at Catamount Health.
Learn More →Personal Wellness Risk Assessment
Not sure where to start with your health journey? Our evidence-based wellness screening tool evaluates five key health dimensions—age-appropriate risk factors, physical activity level, chronic condition status, screening history, and stress burden—to generate a personalized wellness profile and recommended care pathway. The assessment is based on the CDC Community Preventive Services guidelines and USPSTF Grade A/B recommendation framework, and takes fewer than three minutes to complete.
After completing the quiz, you will receive a custom risk score along with specific screening recommendations, program referrals, and next steps tailored to your individual health profile.

Sliding-Scale Savings to Patients
What Our Community Says
“After losing my job and insurance, Catamount Health was the only place that didn't turn me away. Their sliding-scale program made it possible for me to manage my diabetes and keep up with my medications. The care coordinators called me every two weeks to check in. That kind of compassion saved my life.”
Chronic Care Management Patient, Barre
“The behavioral health team at Catamount truly understands rural mental health. My therapist helped me work through severe postpartum anxiety with a combination of CBT and the mothers' support group. I never felt judged, and the fact that my OB visits and counseling were in the same building made everything so much easier.”
Maternal Wellness & Behavioral Health Patient, Montpelier
“I started going to Catamount for a routine wellness exam and ended up discovering I had dangerously high cholesterol. They connected me with a dietitian, set up a walking plan, and within six months my numbers had dropped forty points. The whole team felt like they were genuinely invested in my health.”
Preventive Care Patient, Waterbury
“As a retired teacher living on a fixed income, affording regular health care felt impossible. Catamount Health's Tier 1 sliding-scale rate brought my visit costs down to something I could actually budget for. The nutrition counselor even helped me find local food resources and taught me how to meal-prep on a tight budget. I am healthier at seventy-two than I was at sixty.”
