Preventive care is the single most effective strategy for reducing long-term health risks, managing chronic conditions early, and improving quality of life. Our free Wellness Risk Assessment helps you understand where you stand and what steps to take next.
Why Preventive Health Screening Matters
According to the Centers for Disease Control and Prevention, chronic diseases such as heart disease, cancer, type 2 diabetes, and chronic obstructive pulmonary disease account for seven of the ten leading causes of death in the United States. Yet the vast majority of these conditions are either preventable through lifestyle modification or manageable through early detection and coordinated clinical intervention. The challenge is that many chronic diseases develop silently over years or decades, producing no noticeable symptoms until the condition has advanced to a stage where treatment options become more limited, more invasive, and far more expensive.
Preventive health screening bridges that gap by identifying risk factors, subclinical markers, and behavioral patterns before they escalate into diagnosable disease. For example, routine blood pressure monitoring can catch stage 1 hypertension years before it contributes to stroke or heart failure. A fasting glucose or hemoglobin A1C test can reveal prediabetic metabolic trends that are fully reversible with dietary intervention and moderate exercise. Depression screening instruments such as the PHQ-9 can identify mood disorders at a point when brief counseling and community support are most effective, well before a crisis develops.
At Catamount Health, we believe that every individual in our community deserves access to this kind of proactive, person-centered care regardless of income, insurance status, or prior medical history. That principle is the foundation of our sliding-scale fee model, our partnerships with Vermont Medicaid and Green Mountain Care, and our commitment to meeting patients where they are, both clinically and geographically, through in-person and telehealth options.
The USPSTF Evidence-Based Screening Framework
Our Wellness Risk Assessment is grounded in the screening recommendations of the United States Preventive Services Task Force (USPSTF), an independent, volunteer panel of national experts in disease prevention and evidence-based medicine. The USPSTF systematically reviews the scientific evidence for clinical preventive services and assigns letter grades reflecting the strength and certainty of that evidence. Grade A and Grade B recommendations represent services for which there is high or substantial certainty of net benefit to the patient and are widely adopted as the standard of care across federally qualified health centers and community health organizations nationwide.
When you complete the assessment below, our algorithm cross-references your self-reported age, activity level, chronic condition history, screening recency, and psychosocial stress indicators against the current USPSTF recommendation matrix. The result is a personalized wellness risk profile that identifies which Grade A and Grade B screenings you may be due for, which behavioral and lifestyle interventions are most likely to improve your long-term outcomes, and which Catamount Health programs align with your specific needs.
